New Patient Info
We welcome you to the Dental Group of Tysons. DGT understands your time is valuable, we have taken the appropriate steps to ensure all of our patients are seen in a prompt manner. The new patient process can be a timely one, which we would rather spend with you, creating your ultimate oral health experience rather than going over paperwork. It's easy, simply fill out the new patient packet on-line, 24 hours a day, on your terms, provided to you in order to expedite your initial visit. If you prefer, you can complete your information and send it to our fax at (703) 448-3938. If you bring your completed forms with you, please arrive 15 minutes early to review your insurance information. We accept most major dental plans with the exclusion of HMOs. Our administrative staff will verify your coverage and we will do everything we can to assist you with your insurance.

Please let us know if you require pre-medication prior to your dental visits. Pre-medication might be necessary if you have a heart murmur, mitral valve prolapse or if you have had a prosthetic joint replacement.
At Dental Group of Tysons we take only the necessary X-Rays, so if you have current X-Rays, ask your previous dentist to send them to you and bring them with you, or have them mailed directly to our office.
Using all of our examination findings, we will develop a treatment program that will insure your mouth remains in optimum condition. Prior to undertaking any treatment program we will discuss all findings and insure your questions are answered fully.

Insurance Information
Do you take my insurance?
Dental Group of Tysons files all indemnity insurance. Indemnity insurance is the type of insurance that allows you to go to the dentist of your choice. We also belong to a large number of PPO (Preferred Provider Option) or DMO (Dental Management Option).
When you call the office to schedule your visit, make sure you inquire about your insurance and if we accept your plan. Have your company insurance information ready when you call for your appointment. In some cases insurance companies do not pay the provider (treating dentist), they only pay the patient. If this is the case, it is your responsibility to pay at each visit. We will file all forms for you. For your convenience we take Visa/MC, checks and cash.
Dental Group of Tysons is not listed or does not accept any HMO's, workers comp., or medical pay due to an accident.
Insurance Policies:
The professional treatment you receive at the Dental Group of Tysons is rendered to you, not the insurance company. You are responsible to us for the obligation of payment of treatment.
Please understand that your insurance policy is a contract between you and your insurance company.
Any problems of non-payment or a delay of payment are your responsibility.
Remember, dental benefits were never meant to determine your dental care, they are to assist the patient in the payment of your treatment choice.
We must have a completed and signed form.
Your are responsible for portions not covered by your policy on the day of service.
Any insurance balance over 60 days old is delinquent and is your responsibility to pay.
Accident Insurance cases will be handled by the patient paying for the treatment at the time of service and your insurance or med-pay will reimburse you. Accident claims can take months to settle and we unable to wait for settlement.
We will always do our best to maximize your insurance benefits. However, ultimate responsibility for payment is yours and financial arrangements must be defined prior to beginning dental treatment.
Policies
The changes that are occurring in the health care industry have placed great demands on our office resources. We have established some policies and protocols to help our office run smooth and thus provide better service to you. Please review these policies and make us aware of any special needs or concerns you may have. We thank you for making our practice your choice for the ultimate oral health care experience.
Financial Responsibility
We deal with many insurance plans, all with different requirements. The intricacies of these plans and policies make it impossible for our staff to know specific details related to such. While we make every effort to assist you in receiving the ultimate oral health experience and the maximum benefits to which you are entitled, it is your responsibility to familiarize yourself with your dental plan policy.
We will file your insurance claim for you. However, you are directly responsible for payment in full of any fees. If your insurance company has not paid your claim within 60 days, you will be responsible for full payment of the remaining balance on your account. We are required to collect, and you are responsible to pay, your designated co-insurance, co-payment, and/or deductible at the time service is rendered.
Past due accounts may be referred to an authorized collection agency for collection. A fee of $45.00 will be assessed to all accounts that are forwarded to such an agency. Reasonable attorney fees incurred in attempts to collect on your account will also be assessed, if applicable.
Accounts that have been referred to an outside agency will be placed on a CASH ONLY basis for any future treatment.
Personal checks returned due to "insufficient Funds" or any other reason are subject to a $25.00 service fee.
After we submit the claim, your insurance company will advise us if there are any charges that are not covered. We issue monthly statements to our patients, and we require payment to be made upon receipt of the statement.
Customarily we do not use the amalgam (silver mercury)_ fillings. Please ask for the insurance estimation information regarding composite (tooth colored) fillings. Many insurance companies do not pay or they down code these fillings doe to exclusions in individual policies. If there is anything we can do to provide you a better understanding of general insurance procedures please let our front office staff know.
Appointment Responsibilities
All patients are seen by appointment only. Emergencies will be worked into our schedule as best possible. We ask that you call in advance so that we may reserve time for you.
It is ultimately the patient's (parent's) responsibility to be aware of an office appointment or if pre-medication is necessary ahead of time. We understand that occasional circumstances prevent you from keeping an appointment. When this happens, we ask you to call us as soon as possible (at least 24 hours in advance) so we may provide you with the next available opportunity.
Our office provides a "no/limited wait" policy to the benefit of our patients. We reserve the right to apply changes to your account if appointments are repeatedly broken. |
